NEW STUDENT REGISTRATION
NEW STUDENT REGISTRATION FOR ALL GRADES ENTERING SMITHFIELD SCHOOLS
New student registrations are held at the administration office located at 49 Farnum Pike by appointment only. Forms necessary for the registration appointment are available at the administration office and must be picked up and completed prior to your appointment. Necessary forms are also available for printing in PDF format below.
Kindergarten registration for the next school year usually takes place in early March, by appointment only, at the elementary school in the district where the student resides. For more information, please contact the administration office at (401) 231-6606 ext. 510. Children entering kindergarten may be screened by calling Child Outreach at (401) 231-6608.
Requirements for Registration:
Birth Certificate
Current Immunization form
Physical form (physical must be within the last 12 months)
Mandatory Lead Screening; only required for children that are less than 6 years of age
Vision Screening; only required for children entering Kindergarten
Proof of Residence:
1. Affidavit (form enclosed in registration packet, must be notarized)
2. Copy of a current Mortgage Statement or Property Tax Bill
OR
A fully Executed Signed Lease Agreement AND Evidence of Ownership from the landlord (Field Card).
3. A copy of a current major utility bill (example Gas, Electric, Telephone)
